What is a PATIENT PORTAL?
A Patient Portal is a secure electronic messaging/online service for communicating with your physician’s office. This can be done from your computer or cell phone at a time that is convenient for you.
Why would I want to use this PATIENT PORTAL?
You will be able to request lab results, test results, copies of your medical records, and request medication refills. You will also be able to ask routine questions of your physician, nurse and/or the Medical Home Care Coordinator. You can ask questions regarding your bill and/ or insurance and even pay your bill on-line.
How do I sign up for the PORTAL?
You must provide your email address to the office or call Brenda Jones, PCMH Care Coordinator @ 438-3424. An invitation to enroll will be sent via that email address provided by you. You will be required to create a password that allows only you and your physician’s staff to view your message. There is no cost to sign up for this service.
ALL COMMUNICATION VIA THE PATIENT PORTAL WILL BE SAFE AND SECURE!
